POSITION SUMMARY – RN-Case Manager/Utilization Review/CDI

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – NY RN license; BLS; 1.5+ years recent case management experience (within past 2 years) in an acute care facility; EPIC experience

PREFERRED QUALIFICATIONS – ED CM Experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8hr days

SYSTEMS – Epic

START DATE – ASAP
